Job summary

Community Bridges Inc. is seeking a CBI Jail Coordinator responsible for delivering treatment and discharge planning services to Gila County Jail Program patients. The role involves assisting inmates with jail release planning and coordinating their entry into behavioral health services.

You will participate in weekly staffing meetings with probation and jail staff to provide updates on each patient and ensure ongoing care coordination. A minimum of 2 years of related experience in behavioral healthcare is required.

Qualifications

• 2 years of related experience in behavioral healthcare, physical health, or substance abuse.

Responsibilities

• Deliver treatment and discharge planning services to Gila County Jail Program patients.
• Assist inmates with jail release planning.
• Coordinate entry into behavioral health services.
