Job summary

Jefferson County seeks a compassionate Navigator Coach to support individuals impacted by the justice system, including those with substance use challenges. You’ll help connect participants with resources, establish goals, and build relationships in a new grant-funded program.

You’ll work with justice partners and community organizations, traveling to meet clients, and assisting with transitions back into the community. Strong empathy and collaboration are essential.

Qualifications

• Bachelor's degree in a related field required.
• Minimum three years of professional human services or related experience.
• Ability to work with diverse populations and in jail/community settings.

Responsibilities

• Perform intensive case management to identify needs via intake, enrollment, home visits, and long-term support.
• Travel to meet clients in jail, courts, shelter, or sober housing.
• Create individualized case plans and coordinate resources with community partners.
• Provide one-on-one coaching with a multigenerational approach.
• Coordinate resources for basic needs including food, clothing and shelter.
• Identify treatment and sober living options and coordinate entry for participants.
