Job summary

Community Counseling Center Inc is seeking a dedicated staff member to deliver support services for inmates at Cape County Jail. The role focuses on re-entry strategies, local resources, and coordination with transfers to prison, within a multidisciplinary team.

Position is funded by a 13-month grant from Missouri Department of Public Safety. Schedule is Monday to Friday, 8:00am–4:30pm, with pay of $19.87 per hour, non-exempt.

Qualifications

• Requires valid driver's license and auto insurance.
• Education: high school diploma or equivalent.

Responsibilities

• Assist Jail Navigator with tracking program goals and grant objectives.
• Review and follow up with inmates based on Mental Health screenings.
• Coordinate transportation for inmates upon release.
• Maintain professional boundaries with inmates and staff.
• Provide inmates with appointment times to local health providers.
• Connect inmates to community resources during incarceration and after release.
• Enroll eligible clients in the ERE program and assist with transition.
• Collaborate with jail medical staff to support inmates' medication needs.
• Establish communication with Department of Corrections for continuity of care.
• Support establishment of mental health services during incarceration and after release.
• Update and maintain community resource packets for released inmates.
• Assist with referrals to inpatient facilities and specialized courts.
• Complete Medicaid and disability benefits applications.
• Provide case management to adults in Cape County Jail.
• Document per clinical policies and procedures.

Job Summary

This position is responsible for the delivery of support staff services and assist in connecting to services for inmates in
Cape Girardeau County Jail. These activities include re-entry strategies that involve utilizing local resources for inmates
being released back into the community, as well as coordinating communication with transfers to prison. This position
functions as a professional staff member of a multi-disciplinary team.

Position funded by a 13-month grant from Missouri Department of Public Safety.

Job Duties

• 1. Assist Jail Navigator with tracking needs for the program and/or grant; including updating spreadsheets for goals of
  reducing recidivism rates and use of force.
• 2. Track, review and follow up with inmates based on Mental Health screening tool.
• 3. Coordinate transportation for inmates upon release or provide transportation as necessary and appropriate to
  meet requirements.
• 4. Maintain positive attitude toward agency and inmates while maintaining appropriate boundaries with inmates and
  staff members.
• 5. Provide inmates with appointment times with local mental and physical health providers, as required.
• 6. Connect inmates to community resources while incarcerated and upon release including referrals to inpatient
  treatment facilities, specialized courts, or other treatment modalities.
• 7. Enroll eligible clients in the ERE program for ongoing support and assist with transition to community outreach
  specialist.
• 8. Collaborate with the jail medical staff(s) and psych provider(s) to support the inmate with needs, specifically
  medications, upon arrival and release.
• 9. Establish a line of communication with the Department of Corrections to provide continuity of care for inmates
  transferring to prison.
• 10. Help establish or reestablish services for mental health, during incarceration and after release, including but not
  limited to admission paperwork, flags, fee agreements, and other documentation and/or by collaborating with the
  treatment team.
• 11. Update, copy, track and maintain community resource packets for all released inmates.
• 12. Assist with referrals to inpatient treatment facilities and/or specialized courts and treatments.
• 13. Complete benefits applications such as Medicaid and disability.
• 14. Provide case management type services to adults incarcerated in Cape County Jail.
• 15. Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per
  agency standards.
• 16. Participate in on call rotation, when necessary and eligible.

Qualifications

• High school diploma/GED/HSE
• Must hold Valid Driver's License (Class E for Missouri or Class D for IL Residents - Obtain on start date) and current continuous auto insurance. Our Benefits:
