Job summary

Center for Human Development, Inc. seeks a Jail Diversion Case Manager to deliver case management services to individuals in local county jail, aligned with individualized treatment plans to reduce recidivism due to mental health symptoms.

The role requires collaboration with the Behavioral Health team in La Grande, Oregon. The position is full-time and non-exempt with a comprehensive benefits package including medical, dental, vision, life/AD&D, LTD, 401(k), and professional development

Qualifications

  • Bachelor’s degree in a behavioral sciences field or combination of at least three years of relevant work, education, training, or experience to qualify as a QMHA.
  • CADC or willingness to pursue certification.
  • Must understand mental health assessment, treatment terminology, and apply treatment plans within scope of practice.
  • Must possess a valid Oregon Driver’s license and be insurable for client service purposes and travel.

Responsibilities

  • Implement activities according to individual treatment plans.
  • Document all program activities.
  • Transport clients as needed.
  • Maintain contact with client’s primary therapist as needed.
  • Participate in monthly clinical supervision.
  • Lead DUII Education class.
  • Monitors clients appointments and progress; maintain case files and reports.
  • Assist clients to find and secure housing.

Job description

Seeking a Jail Diversion Case Manager who will work with the Behavioral Health team to deliver case management services to individuals in local county jail as defined and prioritized within the individual treatment plan to reduce the likelihood of recidivism due to persistent mental health symptoms.

Wage: $25.51 - $32.26 hourly. With CADC $26.80 - $34.09.

Experience /Qualifications:

Bachelor’s degree in a behavioral sciences field or a combination of at least three years of relevant work, education, training, or experience required to qualify as a QMHA. CADC or willingness to pursue certification. Have the competencies necessary to: communicate effectively; understand mental health assessment, treatment and service terminology and to apply the concepts and provide psychosocial skills development and to implement interventions prescribed on a Treatment Plan within the scope of his or her practice. Must possess a valid Oregon Driver’s license and be insurable for client service purposes, travel between business offices, and to attend required meeting and/or trainings. Must pass all criminal history check and drug screening.

Essential Job Functions:
  • Implement activities according to individual treatment plans.
  • Document all program activities.
  • Implement and conform to general procedural formats; i.e., confidentiality, personnel, safety, etc.
  • Transport clients as needed.
  • Maintain contact with client’s primary therapist as needed.
  • Participate in monthly clinical supervision.
  • Provides individual and group training in such areas as independent living skills, socialization, vocational skills and money management.
  • Provides support and information to family members including individual consultation, support/education groups.
  • Assists clients in applying for and obtaining services and benefits.
  • Assist clients in community involvement.
  • Lead the DUII Education class.
  • Serves on advisory boards and committees concerned with mental illness.
  • Monitors clients appointments and progress; maintains accurate case notes, treatment plans and client files; completes reports as required in a timely way.
  • Provides supportive and problem-solving counseling to clients and their families; responds to crisis situations.
  • Have the ability to work effectively as a member of a team.
  • Collaboration with providers, gathering information form provider.
  • Maintain accurate and timely records.
  • Assist client’s to find and secure housing.
  • Must maintain punctual and regular work attendance.
  • Must have the ability to maintain positive and professional working relationships with co-workers, clients and other agencies.
  • Communicate effectively both verbally and in writing.
  • Work effectively with and establish rapport with clients.
  • Prepare clear and concise reports; maintain accurate case notes and client files.
  • Establish and maintain effective and cooperative working relationships with other programs, other agencies and the general public.
  • Observe and record behavioral changes.
  • Work independently and as a team member.
  • Willingness to work flexible hours.
  • Other duties as assigned.
