JOB SCOPE:

Responsible for providing employment services to ACT consumers, such as job development and job coaching. Will spend 50% of their time in the community scouting establishments for employment opportunities for ACT consumers and the remaining time providing individual and group counseling, with individuals who are in or preparing to enter the workforce. Will work on a multi-disciplinary team to provide a full range of employment services, including skills acquisition.

ESSENTIAL FUNCTIONS:

1. Provides employment services to consumers.
2. Participates in assessment and treatment planning for consumers, particularly surrounding employment goals and needs.
3. Provides individual and group counseling focusing on reducing functional barriers to employment and enhancing skill development.
4. Maintains case records including completing progress notes and treatment plans.
5. Ensures all relevant case-related information is input into case files.
6. Participates in morning meetings with the ACT team to discuss consumers.
7. Develops program linkages and support services for consumers.
8. Networks and develops jobs in the community, building partnerships with potential employers.
9. Facilitates rapid job placement.
10. Participates in trainings and other required assignments with the Center for Practice Innovation (CPI).
11. Enters consumers who express interest in employment in the New York State Employment Services System (NYESS).
12. Maintains up-to-date employment records in the agency’s EHR and relevant regulatory systems/portals.
13. Provides ongoing employment supports following job placement.

EDUCATION AND EXPERIENCE:

1. BA Degree in a Mental Health Discipline or related field.
2. Minimum of 1 year experience working in a social service agency.
3. Knowledge of community resources available to the population(s) served.
