Responsibilities

• Facilitates care coordination with local businesses to obtain employment for clients.
• Maintains positive relationships with social services partners.
• Provides vocational services: applications, resumes, interview skills, and job-readiness.
• Attends supervision and treatment team meetings weekly.
• Develops individual employment plans within 14 days of referral.
• Conducts on- and off-site job coaching and advocates for accommodations.
• Collaborates with SSA and benefit planners to maintain benefits while working.
• Keeps up-to-date with local job market and builds new contacts weekly.
• Ensures quality documentation and required trainings.
