Job summary

Elizabeth Layton Center is seeking an IPS Supported Employment Case Manager to join our Outpatient team in Paola, KS. The IPS Case Manager will provide case management services to consumers with mental illness who are seeking stable employment and will coordinate with therapeutic staff to connect consumers to employment opportunities.

Qualifications

• Provides case management services to consumers with mental illness seeking stable employment.
• Coordinates with therapeutic staff to connect consumers with employment opportunities.
• Maintains relationships with local businesses and social services partners.
• Communicates effectively, both orally and in writing, with clients and stakeholders.

Responsibilities

• Facilitates care coordination between client and local businesses to obtain employment.
• Builds and maintains positive relationships with social services and employers.
• Provides vocational services: resume writing, interview skills, appropriate employment dress, and interpersonal skills.
• Attends supervision and treatment team meetings to support clients’ employment goals.
• Develops an individual employment plan within 14 days of referral to IPS program.
• Conducts job coaching on/off-site and advocates for reasonable accommodations.
