Job summary

Brian’s House is seeking a Certified Investigator - Incident Management to conduct investigations in the Exton, PA area. The role is full-time with a day shift and collaboration with Administration and HR. This position is hourly non-exempt and reports to the Incident Manager.

The investigator will interview individuals, staff and witnesses, collect evidence, and complete investigation reports for administrative review, while maintaining confidentiality and regulatory compliance.

Qualifications

• Bachelor’s Degree required.
• Must hold Pennsylvania Certified Investigator and maintain certification.
• Strong interviewing, analytical, and report-writing skills.
• Two years’ experience with Intellectual or Developmental Disability (IDD) or Mental Health population.
• On-call availability for nights/weekends/holidays.
• Valid driver’s license and travel to incident locations as needed.
• Knowledge of PA DHS Incident Management system (EIM) preferred.

Responsibilities

• Investigate assigned incidents as mandated by regulations.
• Follow policies on documentation and confidentiality.
• Plan scope and direction with Initial Reporter and Incident Manager.
• Conduct investigations timely, thorough, and objective.
• Conduct wellness checks and interviews; obtain witness statements.
• Perform scene visits and collect physical evidence.
• Review and collect documentary evidence (reports, videos, records).
• Prepare findings for Administrative Review Committee and HR.
• Input information into the EIM database per requirements.
• Maintain confidentiality and professional relationships.
• Assist with incident management reporting and trainings.
• Participate in Incident Management Review Committees.
