Qualifications

• RN licensure in NM or by reciprocal agreement with NM.
• Minimum one year of directly related nursing experience.
• Knowledge of patient access barriers and care coordination.
• Experience with electronic medical records and multidisciplinary teams.

Responsibilities

• Adhere to Patient Centered Medical Home/Patient Centered Specialty Practice expectations.
• Ensure quality customer service; train, monitor, and coach staff.
• Work to eliminate access barriers and assist patients with community resources and financial assistance.
• Collaborate with medical providers and community resources to meet treatment goals.
• Identify and address problems; report needs to leadership.
• Follow hospital and department policies; participate in committees and meetings.
• Maintain and report program statistics; document encounters in EMR.
