Job summary

UNM Hospital in Albuquerque is seeking an RN Nurse Navigator to support Radiology - Interventional Rad. The role focuses on reducing barriers to access, coordinating resources, and delivering positive financial impact while maintaining high-quality patient care.

The successful candidate will have 1 year of related experience, NM RN licensure, CPR/BLS credentials, and strong communication, organizational and advocacy skills to assist diverse patient populations across age groups.

Qualifications

• RN Nurse Navigator role requires knowledge of the department specialty and broad healthcare system understanding.
• Essential: 1 year directly related experience.
• Credentials include CPR for Healthcare/BLS Prov or Prof Rescuers within 30 days; NM RN license or reciprocity as allowed by NM.

Responsibilities

• PATIENT CENTERED MED - Promote Patient Centered Medical Home / Specialty Practice as applicable.
• CUSTOMER SERVICE - Ensure quality service through training, monitoring, and coaching; resolve issues.
• NAVIGATION/PATIENT ADVOCACY - Eliminate barriers to access to culturally competent medical services; help access resources and financial assistance.
• COLLABORATION - Work with medical providers and community resources to meet treatment goals.
• PROBLEM SOLVE - Identify causes and take corrective action; inform leadership of issues.
• POLICIES - Follow hospital, nursing division and departmental policies.
• STATISTICS - Maintain and report program and client service statistics.
• DOCUMENTATION - Document all client encounters in EMR as instructed.
• COMMITTEES/MEETINGS - Attend meetings and participate in hospital-wide activities.
• DEVELOPMENT - Seek professional growth through education and workshops.
• PATIENT SAFETY - Follow patient safety policies and report concerns.
