Soap Creek Resources is seeking an Internal Medicine Physician (MD/DO) for a full-time, in-person outpatient role in East St. Louis, IL.

You will provide comprehensive care to adult patients, manage acute and chronic conditions, and support preventive services and immunizations within a multidisciplinary team. The position emphasizes patient-centered care, population health, and collaboration with clinicians to improve community health outcomes.

Qualifications

• MD or DO from an accredited medical school.
• Board Eligible or Board Certified in Internal Medicine.
• Active state medical license (or ability to obtain).
• Experience in outpatient or community-based care preferred.
• Strong clinical judgment, communication skills, and team-oriented approach.
• Commitment to providing high-quality care to underserved or diverse populations.

Responsibilities

• Provide comprehensive outpatient internal medicine care to adult patients
• Diagnose, treat, and manage acute and chronic medical conditions
• Deliver preventive care, health screenings, immunizations, and wellness services
• Develop and manage individualized treatment plans and follow-up care
• Collaborate with interdisciplinary clinical teams and coordinate specialty referrals
• Deliver culturally competent care with consideration of social determinants of health
• Participate in quality improvement initiatives, care coordination, and population health efforts
• Accurately document patient encounters in the electronic health record (EHR)
• Engage in clinical meetings, care team collaboration, and community health initiatives

Job Type: Full-time, Monday–Friday, daytime schedule, in-person (outpatient)
