Job summary
NOMS Healthcare in Lorain, OH seeks a Nurse Practitioner to provide comprehensive, patient-centered care in a collaborative practice. You will assess, diagnose, treat, and manage patients across conditions in partnership with physicians and interdisciplinary teams.
Responsibilities include comprehensive assessments, order and interpret tests, develop treatment plans, and educate patients. Strong knowledge in care coordination, documentation, and HIPAA compliance is essential.
Qualifications
- Strong clinical assessment, diagnostic and treatment planning skills.
- Proficient in patient education and engagement.
- Experience collaborating with physicians and care teams.
- Proficient with electronic medical records (Epic preferred).
Responsibilities
- Perform comprehensive health assessments and medical history reviews.
- Order, interpret, and evaluate labs, imaging and other diagnostics.
- Develop, implement, and manage patient treatment plans.
- Diagnose and manage acute and chronic conditions within scope.
- Prescribe medications per state regulations and protocols.
- Educate patients and families on diagnoses, treatments, and prevention.
- Maintain accurate medical records in the EMR.
- Collaborate with care teams to coordinate patient care.
