Job summary

Lorain County Health & Dentistry in Lorain, OH is seeking a Family Nurse Practitioner to deliver primary care in our community clinic. This full-time role supports the PCMH/NCQA model and focuses on building long-term relationships with patients while coordinating with the provider team.

The ideal candidate holds an MSN with AP certification, is board certified, licensed in Ohio, and committed to compassionate, patient-centered care. Bilingual Spanish is a plus.

Qualifications

• MSN with Advanced Practice Certification required.
• Must be board certified in area of practice.
• Must have BLS certification.
• Must be licensed in the State of Ohio and meet credentialing requirements.
• Maintain continuing education per State Board requirements.

Responsibilities

• Provide primary clinical care per PCMH/NCQA model in a clinic.
• Review patient history and develop treatment plans.
• Prescribe medications and order tests as needed.
• Collaborate with the provider team and participate in peer review.
• Educate patients and build long-term relationships to support health.
