Job summary

Lorain County Health & Dentistry seeks a Family Nurse Practitioner to deliver primary care in a clinical office setting in Lorain, OH. You will collaborate with physicians, manage long-term patient relationships, and provide patient-centered care within the PCMH/NCQA model.

Requirements include an MSN with APRN certification, board certification, and Ohio licensure, plus BLS. Experience in community health and MAT/MOUD is a plus; bilingual skills are a bonus. Comprehensive benefits offered.

Qualifications

• MSN with Advanced Practice Certification required.
• Board certification in area of practice.
• BLS certification required.
• Must be licensed in Ohio and credentialing ready.
• Maintain continuing education as required.

Responsibilities

• Review patient medical history and current conditions.
• Perform exams, screenings, and routine checkups.
• Order and interpret tests and imaging; review results.
• Diagnose and define treatment plans; prescribe medications as needed.
• Educate patients on health plans and preventive care.
• Collaborate with the provider team and participate in peer review.
