Job summary

Current ACLS/BLS certification is required, and wound-care experience is a plus.

Qualifications

• Board-certified NP or PA with current licensure.
• ACLS/BLS certification required.
• Experience in internal medicine or primary care preferred.

Responsibilities

• Manage established follow-up visits for stable patients.
• Provide chronic disease management (hypertension, diabetes, hyperlipidemia).
• Conduct post-discharge follow-ups and same-day visits when needed.
• Document accurately in the EMR and coordinate care with the team.
