The Internal Medicine Advanced Practice Provider (APP) supports the Internal Medicine physician team by increasing patient access, improving continuity of care, and optimizing physician schedule capacity. The APP manages appropriate established-patient follow-up visits, routine chronic disease management, transitional care visits, and same-day or acute needs within the APP’s scope of practice. The position works collaboratively with Internal Medicine physicians and the clinical care team to ensure patients receive timely follow-up while allowing physicians to maintain schedule capacity for new patients, medically complex patients, and visits requiring physician-level evaluation.
