Job summary

Universal Health Services is seeking staff to assist in case management and psychosocial assessments within a behavioral health setting. The role includes documenting in clinical records, supporting group, family, and individual therapy, and coordinating Master Treatment Plans with the Treatment Team.

The position involves assisting in staff training, developing in-service content, and helping with the implementation of treatment plans to ensure timely care for patients across the facility.

Responsibilities

• Assist in case management for each patient, including contacting referring agencies and documenting within deadlines.
• Assist in psychosocial assessment documentation within required timeframes.
• Document patient behavior and intervention effectiveness in the clinical record daily.
• Assist in facilitating group, family, and individual therapy sessions and document in progress notes.
• Review and present Master Treatment Plans to the Treatment Team per facility and contract guidelines.
• Attend Treatment Team meetings and treatment planning meetings to support ongoing Treatment Plan development.
• Support in researching, developing, and presenting in-service training for staff.
• Help train milieu staff to conduct Skills Groups (Independent Living, Assertiveness, Social Skills, Problem Solving).
• Assist in completing and charting the Initial Master Treatment Plan and implementing the Preliminary Treatment Plan.

Responsibilities

• Assists in the provision of case management for each patient assigned to his/her caseload to include contact with the referring agency as required by guidelines. Complete required case management documentation within required timelines.
• Assists in the Research, performance and documentation of the psychosocial assessment of all patients within specific time frames.
• Assists in the documentation in the clinical record of patient behavior, and interventions provided and their effectiveness on a daily basis, using the format approved the facility
• Assists in the facilitation of group therapy, family therapy, and individual therapy sessions and documents appropriately in the interdisciplinary progress notes.
• Assists in the review and presentation of Master Treatment Plan s of all patients in his/her caseload to the Treatment Team within the guidelines established by the facility, JCAHO, Medicaid, DHEC, state contracts and private insurance.
• Attends Treatment Team meetings with other therapists and attends treatment planning meetings to assist with the ongoing development of the patient’s Treatment Plan.
• Assists in the Research, development, and presentation of in-service training to staff to aid in program development and to increase others’ job knowledge.
• Assists in training milieu staff to conduct Skills Groups covering topics as Independent Living, Assertiveness Training, Social Skills, and Problem Solving. Also co-leads Skills Groups.
• Assists in the Completion and charting of the Initial Master Treatment Plan for each patient in his/her caseload within required timeframes and admission.
• Assists in the Implementation the Preliminary Treatment Plan within required time frames of the patient’s admission.
