Job summary

Universal Health Services' Coral Shores Behavioral Health in Stuart, FL, seeks a licensed social worker to join a 24/7 inpatient team delivering psychosocial assessments, treatment planning, and family involvement.

You will provide individual, group, and family therapies, coordinate discharge planning, document care, and participate in interdisciplinary rounds to support patient and family needs.

Qualifications

• MSW or master's degree in a recognized mental health field.
• BLS certification required.
• Experience with psychosocial assessments and treatment planning preferred.
• Ability to conduct individual, group and family therapies.

Responsibilities

Responsibilities

Come make a difference with us at Coral Shores Behavioral Health. We are located in Stuart Florida.

Coral Shores Behavioral Hospital is an 80-bed behavioral health facility located in Stuart, FL, and provides inpatient treatment for children, adolescents, adults, and senior adults. Our professional staff individualizes care to uniquely fit the intensity and severity of each patient case and their family's needs. Because crisis occurs at all times of the day and night, our services are provided 24 hours a day, 7 days a week.

• Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus by interviewing the patient and their family members.


• Participate in the development and implementation of treatment plans, ensuring that concise, realistic care plans are carried out.


• Provide individual, group and family process-oriented therapies as well as conduct other didactic groups using various professional treatment modalities.


• Document treatment provided to patient in medical record legibly and in a timely manner.

• Develop and coordinate an individualized discharge plan for the patient by utilizing the treatment team and chart information to determine aftercare needs.Identify and assess family or community resources such as group homes, or mental health practitioners to meet patient’s aftercare needs.

• Conduct and attend regular treatment team meetings to provide social work perspective to total case management of the patient with the interdisciplinary team by presenting patient’s case and progress towards treatment plan goals, making treatment recommendations, and actively coordinating each case with all members of the treatment team.
