Deborah's Place in Chicago is seeking a Case Manager to provide direct clinical and case management services to tenants, including intake, assessment, service planning, crisis intervention, individual counseling, and group activities aimed at helping tenants reach housing, income, and health goals.
You will link tenants with resources, coordinate referrals, and support them through eviction-prevention efforts while maintaining professional boundaries and participating in ongoing supervision.
This position is responsible for providing direct clinical and case management services, including: intake and assessment with new tenants, collaborative development of individual service plans with tenants, crisis intervention, individual counseling and providing therapeutic group activities to help tenants meet their goals. The case manager provides linkages and coordinates resources/referrals. Case managers assist tenants in goal attainment, including but not limited to: maintaining their housing, obtaining and maintaining sustainable income, and achieving health and wellness goals.
Education: Bachelor's Degree in Social Work or related field. Master's preferred.
Licensure and/or Certification: LCSW preferred
Job Experience: 2-3 years of case management experience
Special Knowledge & Skills: Knowledge of issues surrounding homelessness, mental illness and substance use; proficiency in Microsoft office software, good oral and written communication skills, good organizational skills, conflict management and crisis intervention skills; demonstrate clear and professional boundaries; act in a professional manner; Valid Illinois driver's license, proof of insurance and must have own vehicle.
