Job summary

Deborah's Place in Chicago is seeking a Case Manager to provide direct clinical and case management services to tenants, including intake, assessment, service planning, crisis intervention, individual counseling, and group activities aimed at helping tenants reach housing, income, and health goals.

You will link tenants with resources, coordinate referrals, and support them through eviction-prevention efforts while maintaining professional boundaries and participating in ongoing supervision.

Qualifications

• Bachelor’s degree in Social Work or related field; Master’s preferred.
• LCSW licensure preferred.
• 2–3 years of case management experience.
• Knowledge of homelessness, mental illness and substance use.
• Proficient with Microsoft Office; strong communication and organizational skills.
• Valid Illinois driver’s license and own vehicle.

Responsibilities

• Provide intake, assessment, service planning, counseling, referrals and advocacy.
• Crisis intervention for mental health or medical emergencies.
• Assist with public benefits applications and advocacy.
• Plan and implement therapeutic group activities.
• Help tenants achieve housing, income and health goals.
• Regularly use motivational interviewing to assess motivation and readiness.
• Engage in clinical supervision, in group and individual formats.
• Address issues to prevent eviction.

Education

Bachelor's Degree in Social Work or related field
