Child & Family Services seeks a Care Coordinator to manage and coordinate services identified in the ICP, reporting to the Senior Care Coordinator and CSA Program Director. The role emphasizes family-centered planning, safety, and ongoing communication across service providers.

The position requires a Master's or Bachelor's degree, MA CANS certification, and a valid MA driver’s license. Evening/weekend availability is expected in this role.

Formación

• Care Coordinators must hold a Bachelor's or Master's degree and have experience navigating child/family-serving systems.
• Must have a valid Massachusetts driver's license and an automobile.
• Certified in the Massachusetts CANS is required.
• Evening/weekend availability is required.

Responsabilidades

• Conducting a comprehensive, home-based assessment including the CANS.
• Developing and implementing a youth- and family-centered ICP with the family and collaterals.
• Creating and updating a risk management/safety plan with the family.
• Maintaining regular face-to-face contact with youth and family and collaterals.
• Coordinating services with state agencies, schools, primary care, and other providers.
• Preparing reports including authorizations, ICPs, and evaluation data.
