Job summary

Allies4Outcomes seeks experienced social workers to join our Adult & Aging Services Intensive Support Social Work Team (ISSWT) across North Carolina. You will partner with county agencies and the A4O Consultant Team to deliver high-quality, results-driven services that support individuals and communities throughout the state.

Ideal candidates have 5+ years as a Social Worker III or in a supervisory role within NC DSS, with strong knowledge of NC policy, practice standards, and quality

Qualifications

• Minimum 5 years of experience as a Social Worker III or in a supervisory role within NC DSS.
• Strong knowledge of NC policy, practice standards, and quality improvement.
• Ability to work independently while leveraging technology effectively.
• Demonstrated adaptability, emotional intelligence, and change management skills.
• Willingness to travel across North Carolina and maintain a flexible schedule.

Responsibilities

• Adult Protective Services (APS): Investigate reports of abuse, neglect, and exploitation; assess need for protective services; develop safety plans.
• Guardianship & Legal Interventions: Initiate and manage guardianship proceedings; prepare legal documentation and represent DSS in court.
• In-Home Aide & Support Services: Assess eligibility and level of care needs; develop and monitor support plans for safe living.
• Long-Term Care & Special Assistance: Evaluate placement needs; determine eligibility for Special Assistance and Medicaid; conduct reassessments.
• Case Management & Collaboration: Coordinate with mental health, housing, healthcare, public health; provide crisis intervention and long-term planning.
