Job summary

CareStar, Inc. seeks an Intensive Support Coordinator to support Walton County, GA and surrounding areas. This full-time role combines remote work with field visits to coordinate care for individuals, link them to needed services, and develop personalized care plans.

As part of CareStar's mission-driven team, you will assess needs, link to medical and social services, and monitor plans. A bachelor’s degree in a human services field and care coordination experience are preferred.

Qualifications

• Bachelor’s degree in a human service field (Social Work, Psychology, Sociology, Counseling, Gerontology, Nursing) or closely related field.
• Two years direct service experience serving persons with DD/IDD or related disabilities.
• Two years’ experience in healthcare, habilitative/rehabilitative, residential, behavioral health or similar setting.

Responsibilities

• Link participants with medical, social and other programs to address identified needs and goals.
• Conduct health, safety, social and environmental assessments per guidelines.
• Participate in workgroups, community groups and committees as requested.
• Demonstrate effective organizational and communication skills.
• Provide care management/coordination when applicable.
• Travel as required (flexible schedule, valid driver's license and car insurance).

Key Responsibilities

• Provides information and assistance to link participants with medical, social and/or
  educations providers and/or other programs and services that can provide needed
  services to address identified needs and achieve identified goals.

• Conducts health and safety, social and environmental assessments per established
  guidelines.

• Serves on work groups, community groups, committees, or other task forces and attends
  meetings as requested.

• Effective organizational and communication skills

• Experience providing care management/coordination preferred.

• Willingness and ability to work flexible schedule to meet needs of families including reliable transportation, valid driver's license and car insurance as required by state law, and the ability to travel.

Minimum Qualifications

• Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.

• Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

• Bachelor’s degree in human service Field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing (or closely related field).

• Two year's direct service experience serving persons with DD/IDD or related disabilities.

• Two years’ experience in healthcare, habilitative/rehabilitative, residential, behavioral health setting (ICF/IDD), psychiatric inpatient hospital/facility, specialty clinic, special education department or similar setting.

• Experience in completing assessments, service plan development and monitoring.

• Effective organizational and communication skills

• Experience providing care management/coordination preferred.
