Intensive Support Coordinator - Fulton and Surrounding Counties

We are seeking an Intensive Support Coordinator in the Clinical department to identify, coordinate, and oversee the delivery of services that enhance the health, safety, and well-being of participants, helping them achieve maximum independence.

THE POSITION

CareStar is looking for motivated individuals who:

• Hold a bachelor’s degree in a human service field such as Social Work, Psychology, Sociology, Counseling, Gerontology, Nursing, or a related field.
• Have two years of direct service experience serving persons with DD/IDD or related disabilities.
• Possess two years of experience in healthcare, habilitative/rehabilitative, residential, behavioral health settings (ICF/IDD), psychiatric inpatient hospital/facility, specialty clinics, or similar environments.
• Have experience in completing assessments, developing, and monitoring service plans.
• Demonstrate effective organizational and communication skills.
• Experience in care management/coordination is preferred.
• Are willing and able to work a flexible schedule, including reliable transportation, a valid driver’s license, and car insurance, and are willing to travel.
• Adhere to the CareStar Rule and comply with company policies and procedures.
• Maintain confidentiality regarding patient information and follow all data protection policies.
• Follow the Acceptable Use Policy for all company information systems.

This position is based in Fulton County and covers surrounding counties.
