CareStar, Inc. is seeking an Intensive Support Coordinator in the Clinical department to coordinate and oversee services that enhance the health, safety, and well-being of participants, helping them achieve maximum independence. The position covers Richmond and surrounding counties.

Responsibilities and Requirements
  1. Bachelor’s degree in human service fields such as Social Work, Psychology, Sociology, Counseling, Gerontology, Nursing, or related.
  2. Two years of direct service experience with persons with DD/IDD or related disabilities.
  3. Experience in healthcare, habilitative/rehabilitative, residential, behavioral health settings, or similar.
  4. Skills in assessments, service plan development, and monitoring.
  5. Effective organizational and communication skills.
  6. Care management/coordination experience preferred.
  7. Willingness to work flexible hours, reliable transportation, valid driver’s license, and ability to travel.
  8. Adherence to CareStar policies, confidentiality, and acceptable use policies.
