Job summary

Youth Haven Services is seeking a qualified professional to provide Intensive In-Home services to children and adolescents in Randolph County, NC. The role emphasizes clinical oversight, case management, and collaboration with therapists, families, and schools to support families in maintaining youths at home and in their communities.

The position is W-2, with compensation based on billable contacts, and requires coordination with licensed therapists to ensure coordinated, quality care.

Qualifications

• Provide Intensive In-Home case management and rehabilitative services to children and adolescents.
• Collaborate with licensed therapists, families, schools, physicians, and community partners to coordinate care.
• Maintain accurate, timely, and compliant clinical documentation including progress notes and service logs.
• Implement Person-Centered Plans (PCPs) and complete NC TOPPS assessments and SARs as required.

Responsibilities

• Complete client intakes, assessments, and PCPs.
• Meet clients in homes, schools, and community settings to deliver case management and skill-building services.
• Monitor client progress and assist families in achieving treatment goals identified in the PCP.
• Maintain required agency paperwork and documentation in a timely manner.
• Coordinate care with therapists, families, schools, physicians, and community providers.
