Youth Haven Services in Randolph County, NC seeks a compassionate Qualified Professional to deliver Intensive In-Home services to children, adolescents, and families. You will provide clinical oversight, case management, and skill-building while collaborating with licensed therapists.

Most work occurs in clients' homes and communities, following Person-Centered Plans, coordinating with families, schools, and providers to ensure coordinated care.

Aufgaben

• Complete client intakes and Person-Centered Plans (PCPs).
• Submit initial authorizations and reauthorizations for Intensive In-Home services.
• Coordinate care with therapists, families, schools, physicians, and other providers.
• Meet clients and families in homes, schools, and communities for case management and skill-building.
• Maintain compliant clinical documentation and required agency paperwork.
• Participate in Child and Family Team (CFT) meetings and other planning sessions.
