Job summary

If you're looking for meaningful work with flexibility and the opportunity to make a real difference, we'd love to hear from you.Position SummaryThe Qualified Professional provides Intensive In-Home case management and rehabilitative services for children and adolescents with behavioral and mental health needs. Services are delivered primarily in the client's home and community and are guided by each individual's Person-Centered Plan. This position works collaboratively with the licensed therapist, family members, schools, and other providers to ensure coordinated, quality care.This is a W-2 position and compensation is based on billable client contacts.Essential ResponsibilitiesComplete client intakes, assessments, and Person-Centered Plans (PCPs).Submit initial authorizations and reauthorizations for Intensive In-Home services.Coordinate care with therapists, families, schools, physicians, and other community providers.Meet with clients and families in their homes, schools, and community settings to provide case management and skill-building services.Monitor client progress and assist families in achieving treatment goals identified in the Person-Centered Plan.Maintain accurate, timely, and compliant clinical documentation, including progress notes, service logs, treatment updates, and required agency paperwork.Complete NC TOPPS assessments, Service Authorization Requests (SARs), discharge documentation, and other state-required documentation.Maintain assigned caseloads and ensure authorizations remain current.Assist families in accessing community resources and support services.Participate in Child and Family Team (CFT) meetings and other treatment planning meetings as needed.Perform other duties as assigned in support of the Intensive In-Home program.CompensationThis position is compensated on a per-contact (billable service) basis.
