Job summary

Youth Haven Services is seeking a compassionate Qualified Professional to provide Intensive In-Home services to children and adolescents in Randolph County, NC. The role emphasizes clinical oversight, case management, and skill-building within the family home and community, collaborating with therapists and partners to help families achieve lasting success.

Work is conducted primarily in clients' homes and local settings, using Person-Centered Plans to guide care.

Qualifications

• Experience providing home- or community-based behavioral health services.
• Ability to develop and monitor Person-Centered Plans (PCPs).
• Strong collaboration with licensed therapists, families, and schools.

Responsibilities

• Complete client intakes, assessments, and PCPs.
• Submit initial authorizations and reauthorizations for IIH services.
• Coordinate care with therapists, families, schools, physicians, and community providers.
• Meet with clients in homes, schools, and community settings for case management and skill-building.
• Monitor progress and assist families to achieve treatment goals.
• Maintain accurate clinical documentation and required paperwork.
• Complete NC TOPPS assessments, SARs, discharge docs, and other state forms.
• Maintain caseloads and ensure authorizations stay current.
• Assist families in accessing community resources and services.
• Attend Child and Family Team meetings and other planning meetings.
