Job summary

Youth Haven Services, LLC seeks a compassionate Qualified Professional to deliver Intensive In-Home services to children and families across Person County, NC. Qualifications

• Bachelor’s degree in a Human Services-related field is required.
• Minimum of two (2) years of documented experience working with children, adolescents, and families receiving behavioral health or mental health services.

Responsibilities

• Complete client intakes, assessments, and Person-Centered Plans (PCPs).
• Submit initial authorizations and reauthorizations for Intensive In-Home services.
• Coordinate care with therapists, families, schools, physicians, and other community providers.
• Meet with clients and families in their homes, schools, and community settings to provide case management and skill-building services.
• Monitor client progress and assist families in achieving treatment goals identified in the Person-Centered Plan.
