Job summary

The Villages of Indiana is seeking a Case Manager – Intensive Foster Care to lead intake, develop treatment and family support plans, and coordinate services with the DCS FCM. You will drive collaborative care and ensure the child’s safety and well-being in a team setting.

You will work closely with foster parents to provide trainings, support groups, respite coordination, and activities, while assisting with behavior management and skill-building for both caregivers and children.

Qualifications

• Bachelor’s degree required.
• BSW with LBSW preferred if applicable.
• Five years of supervised, paid casework experience, two of which in an approved child placing agency.
• Graduate training in social work may substitute for up to five years of experience.

Responsibilities

• Handle intake and develop Treatment Plan and Family Support Plan.
• Coordinate services with team members to determine needed services.
• Be main contact with the DCS FCM and attend CFTM meetings as needed.
• Support foster parents by coordinating trainings, support groups, respite, and activities; assist with behavior management and skill building.
