Job summary

Indiana Mentor, part of the Sevita family, is hiring a Case Manager Foster Care to support youth in Indianapolis and surrounding areas. Join a mission-driven team that provides residential and treatment foster care services with a focus on stability and well-being.

You will coordinate services, monitor quality of care, collaborate with families and external case managers, and help develop plans that meet each individual's needs.

Qualifications

• Bachelor’s degree in human services or related field is required.
• At least one year of experience in child welfare.
• Current driver’s license and reliable transportation.
• A compassionate approach and commitment to quality in everything you do.

Responsibilities

• Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
• Oversee all services provided to individuals and their families, monitor quality of care, ensure contractual compliance and regulatory program compliance.
• Assist in developing quality services and engaging activities that meet individuals’ needs.
• Build and maintain relationships with families and external case managers.
