Job summary

The Villages of Indiana is seeking a Case Manager – Intensive Foster Care to lead intake, assess needs, and develop comprehensive Treatment and Family Support Plans. You will coordinate services with the team and act as the primary liaison with the DCS FCM, attending CFTM meetings as required.

Qualifications

• Bachelor’s degree from an accredited four-year college is required.
• If the candidate holds a BSW, an LBSW is required.
• Five years of supervised, paid casework experience, with two years in an approved child placing agency.
• One year of graduate training in social work may substitute for part of the required experience.

Responsibilities

• Handle intake and implement assessments to develop the Treatment Plan and Family Support Plan.
• Coordinate services with team members to determine needed supports.
• Serve as the main contact with the DCS FCM and attend CFTM meetings as needed.
• Support foster parents by coordinating trainings and support groups, arranging respite and extracurricular activities, assisting with behavior management, and building skills for families.
