Job summary

Riverview Psychiatric Center is recruiting an Intensive Case Manager to work in the social work department. This professional role provides intensive case management for high-risk mental health clients, guiding their transition from hospital to the community and ensuring basic living needs are met.

Applicants must have a Bachelor's in Social or Behavioral Sciences with two years of relevant professional experience.

Qualifications

• Bachelor’s Degree in Social or Behavioral Sciences with two (2) years’ experience in professional community mental health work.
• A Master’s Degree in Social Work may be substituted for two (2) years’ experience.

Responsibilities

• Provide intensive case management for high-risk mental health clients.
• Support transition from hospital to community and coordinate access to medical, psychiatric, rehabilitation, social and educational services.
• Monitor client progress and overall service delivery system to ensure needs are met.
