Job summary

Merakey is seeking an Intensive Case Manager for Adults in Carlisle, PA. The role focuses on delivering ICM services to adults with serious mental health needs, including assessments, service plans, and ongoing support to improve outcomes.

Qualifications

• Bachelor's degree or equivalent in relevant fields.
• Experience in human services with client contact preferred.
• RN license is acceptable.

Responsibilities

• Travel throughout Cumberland and Perry Counties to meet clients in their homes and communities.
• Deliver Intensive Case Management (ICM) services to adults with serious mental health needs.
• Conduct assessments, develop service plans, and provide ongoing support through linking, monitoring, advocacy, and skill-building.
• Support clients in accessing psychiatric care, community resources, and social supports.
• Provide education and support to families and significant others.
• Maintain accurate and timely documentation in compliance with Merakey Behavioral Health, County, and State regulations.
• Participate in clinical supervision, staff meetings, and quality improvement initiatives.
• On-call for caseload.

Education

Bachelor's degree in sociology, social work, psychology, or related field

High school diploma / GED with 12 college credit hours + 2 years human services experience (1 year direct client contact)

Registered nurse (RN) license

Job description

The ideal candidate will possess the following qualifications:

• Bachelors degree in sociology, social work, psychology, or a related field; OR high school diploma/GED with 12 college credit hours completed in relevant coursework, plus two years experience in human services with one year in direct client contact; OR a registered nurse (RN) license.
• Valid driver's license and reliable transportation required.
• Experience working with adults in public or private human services is preferred.
