Job summary

Merakey is seeking a full-time Case Manager to join the Dual Diagnosis Treatment Team in Greensburg/Indiana, PA locations. You will assess, plan, and transfer recovery skills for adults and youth with mental health diagnoses and/or on the autism spectrum in various settings, using a person-centered approach.

Join a team that promotes growth within Merakey's Adult Behavioral Health Program. Valid PA driver’s license and behavioral health experience are preferred as you deliver intensive services

Qualifications

• Bachelor's degree in social work, psychology, or another human service-related field required.
• Valid PA driver's license with a clean driving record and reliable transportation required.
• Experience in the behavioral health field working with individuals with mental health/developmental disabilities.

Responsibilities

• Assess, plan, and transfer recovery treatment and support plans for adults and youth with mental health diagnoses and/or autism spectrum conditions.
• Provide intensive therapeutic services in diverse settings, collaborating with the treatment team to implement person-centered plans.

Position Details

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. This candidate, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.
