• Providing resource identification, referral services, and consultation on various health and social needs.
• Serving as a liaison among members, families, providers, and community organizations for healthcare and placement decisions.
• Assisting with HCBS navigation, supplies, and equipment coordination.
• Conducting social work consultations and resource development.

Collaborates with primary care physicians and decision makers to develop personalized service plans, facilitating access to medical care and community-based services. Coordinates Home Community Based Services (HCBS), communicates with healthcare teams and community agencies, and supports families in crisis. Promotes EPSDT compliance.
