Job summary

AlohaCare in Honolulu seeks a Health Coordinator to conduct face-to-face and virtual assessments, develop personalized Health Action Plans, and collaborate with members, providers, and physicians to coordinate primary, acute, behavioral, and LTSS services.

The role includes monitoring progress, coordinating care across the continuum, ensuring HIPAA compliance, and guiding members through social and health services while meeting quality metrics and regulatory standards.

Qualifications

• Degree in health care related field or equivalent experience.
• Experience coordinating care for individuals with complex needs.
• Ability to work with diverse populations and providers.

Responsibilities

• Conduct face-to-face or virtual health assessments and develop action plans.
• Coordinate primary, acute, behavioral, and LTSS services with providers.
• Monitor member progress and ensure plans are implemented within 30 days.
• Ensure HIPAA compliance and maintain confidentiality in care coordination.
