Qualifications

• Minimum one year of social work experience.

Responsibilities

• Conducting comprehensive assessments and creating client-focused service plans within 15 days of notification.
• Maintaining accurate documentation in electronic health records, updating progress, assessments, and care changes.
• Providing resource identification, referral services, and consultation on various health and social needs.
• Serving as a liaison among members, families, providers, and community organizations for healthcare and placement decisions.
• Assisting with HCBS navigation, supplies, and equipment coordination.
• Conducting social work consultations and resource development.

Education

Master’s degree in social work
