Job summary

MultiCare Rockwood Clinic seeks an Integrated Mental Health Therapist to join a multidisciplinary primary care team. You will provide evidence-based behavioral health services within the primary care setting, partnering with a Psychiatric Consultant and PCP to promote symptom reduction and enhanced patient outcomes.

The role emphasizes assessment, treatment planning, and collaborative care with ongoing monitoring and timely care plan adjustments.

Qualifications

• Master's degree in Social Work, Counseling, Marriage & Family Therapy, or related field.
• Licensed credentials such as LMFT/LMHC or LICSW are required or preferred.
• Minimum two years of experience in a behavioral health setting.

Responsibilities

• Assess patient symptoms and create customized treatment plans.
• Screen for behavioral health disorders using standard assessment tools.
• Monitor symptoms and adjust treatment to reduce symptoms.
• Support psychotropic medication management in collaboration with PCPs.
• Track patient progress using the clinic registry and follow-up outcomes.
• Facilitate care plan changes and appropriate referrals as needed.
• Manage behavioral health crises within a primary care setting.
• Provide flexible services including warm handoffs during medical visits.
