Job summary

MultiCare Rockwood Clinic seeks an experienced Licensed Integrated Mental Health Therapist to provide assessments, treatment planning and psychosocial therapy within a primary care setting. Work with a multidisciplinary team in a Rockwood Clinic environment to support diverse patients across preventive and chronic care pathways.

Qualifications

• Master’s degree in Social Work, Counseling, LMFT, LMHC, or related field.
• Licensed LMFT, LMHC or LICSW per HB 1724 are required or held.
• Minimum two years of experience in a behavioral health setting.

Responsibilities

• Assesses patient's symptoms and creates customized treatment plans.
• Screens patients for common behavioral health disorders using PHQ9, GAD7, Columbia Suicide Risk Assessment, or equivalent tools.
• Monitors changes in symptoms and side effects with goal of reducing symptoms.
• Supports psychotropic medication management prescribed by PCPs, focusing on adherence and efficacy.
• Tracks patient follow-up and outcomes using a registry to re-engage patients.
• Facilitates care plan changes including medications or referrals when not improving.
• Manages behavioral health crises in a primary care setting.
• Provides flexible services via scheduled appointments and urgent warm handoffs during visits.

Education

Master’s degree in Social Work, Counseling, LMFT, LMHC, or related field
