Job summary

MultiCare Auburn Medical Center seeks an Licensed Integrated Mental Health Therapist to provide assessments, treatment planning and psychosocial therapy in a primary care clinic. You’ll work with a multidisciplinary team to promote symptom reduction and patient outcomes, using evidence-based interventions and strong communication within the care team.

The role requires a Master’s degree in a related field, a licensed mental health credential (LMFT/LMHC/LICSW) per HB 1724, and at least two years

Qualifications

• Master's degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field
• A Mental Health Professional as defined within HB 1724 one of these higher credentials are held: LMFT, LMHC, LICSW
• Minimum two (2) years of experience in a behavioral health setting

Responsibilities

• Assesses patient's symptoms and creates a customized treatment plan based on individual needs
• Screens patients for common behavioral health disorders using PHQ9, GAD7, and other tools as required
• Monitors changes in clinical symptoms and treatment side effects/complications to reduce symptoms
• Supports psychotropic medication management prescribed by PCPs, focusing on adherence and effectiveness
• Tracks patient follow-up and outcomes using a registry and re-engage patients
• Facilitates care plan changes for non-responders and refers for additional services as needed
• Manages behavioral health crises as they arise in a primary care setting
• Provides flexible behavioral health services to patients on/off current caseload during visits
