Job summary

Betty Jean Kerr People’s Health Centers is seeking a Co-Occurring Community Support Specialist to assist individuals at risk of hospitalization or recently discharged to stay engaged in community living. You will conduct assessments, participate in treatment planning, and coordinate supports with families and services.

The role requires a Bachelor’s in Human Services or equivalent experience, with strong interpersonal skills, documentation, and collaboration in a multidisciplinary team.

Qualifications

• Bachelor's degree in Human Services or related field from an accredited school, OR two years of higher education with two years of related experience.
• Experience in psychiatric, substance abuse treatment, or developmental disabilities preferred.

Responsibilities

• Assess client status including strengths, progress, problems and needs.
• Participate in assessment, development and review of individualized treatment plan.
• Monitor client participation and progress in treatment programs.
• Provide 24-hour resource access to public services (financial, medical, housing, emergencies).
• Advocate for clients within the community to achieve and maintain adjustment.
• Maintain contact with hospitalized clients and participate in discharge planning.
• Train clients in daily living skills (housekeeping, cooking, grooming).
• Assist clients in creating personal support systems including family or guardians.
• Participate in multi-disciplinary teams for staffing.
• Complete documentation on all aspects of community support: case opening, referrals, progress notes, discharge planning, case closure.
• Participate in staff training to learn interviewing, assessment, rapport, treatment planning, and knowledge about mental illness.
• Perform other related duties as required.
