Job summary

Betty Jean Kerr Kerr Peoples Health Centers in St. Louis, MO seeks a Youth Community Support Specialist to provide activities and interactions that help individuals at risk of hospitalization maintain community living.

The CSS evaluates client status, participates in person-centered plan development, and monitors progress toward goals. Responsibilities include transporting clients, making house visits, and recording progress within the Clinical Treatment Team.

Qualifications

• Bachelor's degree and two years of qualifying experience, or four-year combination of higher education and qualifying experience.
• Qualifying experience must include delivery of service to individuals with mental illness, substance use, or developmental disabilities.
• Experience must include one-on-one or group services with teaching and modeling for coping and management.
• Must have valid Class E driver's license, reliable transportation, and vehicle insurance.

Responsibilities

• Provide activities and interactions to help individuals at risk of hospitalization maintain community living.
• Evaluate client status, participate in development of person-centered plans, and monitor progress toward goals.
• Transport clients to appointments and conduct house visits as applicable.
• Maintain client records and uphold standards within the Clinical Treatment Team.
