Job summary

Valley Comprehensive Community Mental Health Center Inc is seeking a Community Engagement Specialist to work with a multidisciplinary team to assess consumer needs, plan and implement treatment goals, and coordinate supports in home and community settings.

You will assist with housing navigation, daily living skills, transportation, and links to community resources, while documenting progress and collaborating with Service Coordinators and crisis teams to prevent unnecessary hospitalizations.

Qualifications

• BA/BS in Human Service Field.
• Six months to one-year experience with working with individuals in human services or related training.
• Valid Driver's License & ability to legally transport clients.
• Ability to comply with all policies and procedures.
• Ability to read, write and speak the English language.
• CPR/First Aid, and MANDT Certifications preferred.

Responsibilities

• Assess consumers needs and evaluate progress.
• Develop nutritious menus; prepare a shopping list of groceries and other household items; transportation to the grocery store.
• Assist individuals in their home to prepare a meal.
• Assist individuals in developing and maintaining a budget.
• Provide outreach to individuals at high risk through crisis services and community agencies.
• Participate in interdisciplinary treatment team meetings (IDTs) and review treatment plans quarterly.
