Job summary

Coastal Horizons is seeking an Integrated Care Therapist to join an on-site team at Cape Fear Clinic – Wilmington, NC. The role focuses on delivering comprehensive behavioral health services within an integrated care model for adults with co-occurring conditions.

Responsibilities include assessments, evidence-based treatment planning, care coordination, and collaboration with medical and behavioral health professionals to support outcomes and engagement, during regular daytime hours.

Qualifications

• Master’s degree in behavioral health required.
• Active licensure required for the position.
• LCSW preferred, or LCSWA/LCMHCA/LCMHC, or equivalent dual licensure.

Responsibilities

• Provide individual, group, and family therapy.
• Conduct psychosocial and diagnostic assessments and develop treatment plans.
• Coordinate care across agency programs and with community partners.
