Qualifications

• Bachelor's degree in health, psychology, sociology, social work, nursing or related field; 2 years of direct experience with individuals with behavioral health conditions.
• Preferred: 1 year experience in care management or coordination; Commissioner for Case Management
• Certification (CCM) trained to provide evidence-based care coordination and clinical assessments.

Responsibilities

• Oversee Care Management services based on standards for enrolled populations.
• Develop comprehensive, patient-centered assessments including biopsychosocial needs.
• Coordinate with clients, caregivers, providers to address barriers and access to care.
• Implement care management interventions, set goals, and plan care during transitions.
• Educate clients/families about diagnoses, medications, and available resources.
• Maintain documentation per policies and HIPAA; provide home visits as needed.
