Qualifications

• Medical Assistant may be considered based on relevant experience.

Responsibilities

• Provide first point of clinic contact for new clients and engage them to access services.
• Coordinate appointments, medications, and other healthcare needs; document tracking as needed.
• Educate clients and support wellness activities such as healthy eating and physical activity.
• Assist clients with navigation to housing, transportation, and community resources to access care.
• Support transitions to outpatient care after discharge to reduce readmissions.
