Qualifications

Responsibilities

• Provide initial clinic contact and engage clients to access services.
• Perform health screenings, nursing assessments and triage healthcare needs.
• Participate in care planning with the team to address client needs.
• Assist clients in navigating the healthcare system to access appropriate care.
• Advocate for clients with healthcare providers and managed care organizations.
• Coordinate access to services addressing social determinants of health.
• Develop knowledge of local providers and collaborate as needed.
• Educate clients and support wellness activities (nutrition, physical activity, healthy behaviors).
• Assist recently discharged clients to transition to outpatient care using navigation services.
