Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or related field.
• At least one year of full-time experience post-Bachelor's working with adults in a human service setting.
• Must have reliable transportation and a valid driver's license.

Responsibilities

• Conducts and documents comprehensive assessment of client and/or client’s needs.
• Participates in development of client’s service plan, consistent with assessment.
• Arranges for delivery of needed services identified in assessment.
• Assists client in accessing needed services through referrals or linkages.
• Monitors client’s progress with referrals, appointments, and follow-ups.
• Performs advocacy activities on behalf of client and coordinates emergency referrals as needed.
• Maintains case records in accordance with SCS standards and regulations.
• Provides case consultation with service providers and collaterals.
• Communicates with supervisor about case management and resource development.
• Provides psychosocial education to clients and/or family members.
